I agree to pay in advance and in accordance with existing ordinances and regulations of the city, the below charges,
the exact charges will be determined and payable immediately upon completion of installation.
I further agree that all rates and charges for water and sewer service to the above property shall be paid in accordance
with existing ordinances and regulations of the city, or any such ordinances and regulations passed hereafter.
I hereby authorize any tenants, to have all accounts for water kept in his/her name with the understanding that water
bills mailed to said tenant shall not relieve the property from liability for water charges incurred.
I understand that the city will use all reasonable effort to maintain uninterrupted service, but reserves the right to shut
off the water at any time without notice for repairs, extensions, non-payment or rates or any other reason and assumes
no liability for any damage as a result of interruption of service from any cause whatsoever.
The sewer superintendent may require the permitee to furnish plans pertaining to the application and issuance of
permit. Reference; Chapter 13.08, Arlington Municipal Code for rules and regulations governing side sewers and
connections to the public sewer system.
SALES TAX NOTICE: Sales tax relating to construction and construction materials in the City of Arlington must be
reported on your sales tax return form and coded City of Arlington #3101.
CONDITIONS
1.) Applicant is required to complete an Utility Extension Agreement Public
Works
2 ) All wells on the property are required to be decommissioned per city standards within 30 days of Public
connecting to city water, with a copy of the well report supplied to the city. Call the water department at Works
360-403-3526 to schedule personnel to witness the cutting & capping of the well line to the house.
3.) All connection fees to be paid prior to water meter installation. Public

4.) Must follow all City of Arlington Public Works Construction Standards and Specifications
5.) Connection to a water or sewer mainline requires 48 hours advance notice, call 360-403-3526
6.) Water Meter Installation or Tail Piece inspection requires 48 hours advance notice, call 360-403-3526
7 ) Well decommissioning requires 24 hour advanced notice, to schedule an inspection call 360-403-3526. City
of Arlington recommends a PRV to be installed to avoid water pressure fluctuations.
8.) Side Sewer Inspection requires 24 hours advanced notice, call 360-403-3508.
9.) Monthly Water/Sewer billing begins on date of water meter installation.
INSPECTIONS
Inspection Line: 360-403-3417 Call for inspections by 3:30 pm for next day inspection. Please allow 48 hours for Fire
Inspections. Please provide the following information when leaving a message: Permit Number, Type of Inspection, and
provide preference of morning or afternoon inspection.
Inspection Portal: Please login to customer portal and submit for the desired inspection type for the permit.
