Washington, Rules and Regulation of the State Board of Health. This permit may be suspended or revoked by the
Health Officer for failure of the Permittee to comply with applicable rules and regulations. Issuance of this permit shall
not be deemed to constitute approval or waiver of noncompliance by the applicant with other applicable governmental
agency requirements, relations, ordinances and laws.

PERMIT GUIDELINES
This permit is valid for the current year for the location and operator noted on the permit and is NOT transferable
upon change of ownership. Post the permit in a conspicuous place at your business location.
If a change of ownership occurs at this establishment, contact the Snohomish County Health Department (SCHD).
New owners must apply for a new permit, including submittal of all required fees, prior to beginning operation.
Permit renewal applications are mailed annually to the mailing address you have provided. Ifyour mailing address
changes, please notify Snohomish County Health Department at 425-339-5250. Failure to renew a permit and/or pay
any outstanding fees prior to the permit expiration date will result in the addition of a late fee and possible facility
closure.
