ELECTRONIC NOTICES CONSENT OR REVOCATION FORM
The Washington Uniform Common Interest Ownership Act(WUCIOA)provides in RCW 64.90.515(3)that any
owner may opt to receive official Association notices electronically (rather than by hard copy mailed via the
U.S. Postal Service) if the owner indicates the preference for electronic notice in writing to the Association,
and designates the address, location, or system to which notices may be electronically transmitted.
With such consent, notices that will be electronically transmitted include all actions for which the Association
is required to provide official notice, including meeting dates/times/locations, proposed amendments to the
CC&Rs, issues to be voted upon without a meeting, implementation or amendment of rules, fines, etc.
Consent to electronic notices applies only to Association communications. Your email address will not be
shared with other owners or entities except for the Association's management company, if any.
To consent to receive electronic notices, or to revoke a previous consent to receive electronic notices,
please complete the form below. **Your signature must be handwritten or digitally encrypted with a
key via a platform such as DocuSign or HelloSign.** Return this completed form by either email or
U.S. Mail to the addresses shown below:
