The inspection was requested by Heidi Berger-Hansen, Construction/Capital Projects Manager for the Arlington Public Schools and conducted by AHERA certified building inspectors from The Kriebel Company, Inc. N V L Laboratories, Inc a NVLAP certified laboratory, analyzed the suspect bulk asbestos sample collected. The following report contains material description, sample location, assessment of asbestos containing material, laboratory results and accreditation, and inspector certification. AHERA Building Inspector: Dan Shelly Inspector Number: 9945-98 Expiration Date: May 3, 2001 Signature SECTION 2 Sample Summary: Asbestos & TCLP 9 ASBESTOS SUMMARY with RECCOMMENDATIONS (Recommendation comments are not to be used as contract document directives.) 8621 186`s Street Arlington, Washington: The trailer is a doublewide one story with a two story wood addition built on the south side. The primary source of heat was baseboard electric heaters and a wood stove. No suspect asbestos thermal system insulation was discovered during the inspection. ASBESTOS: Multi layers of flooring(homogenous 901)were sampled from the kitchen floor. The checkerboard pattern floor tile and mastic were found to contain no detectable asbestos. The sheet vinyl flooring underneath the floor tile was found to contain 55% chrysotile asbestos. MATERIAL AREAS AMOUNT ASSESSMENT DAMAGE Multi layers of Kitchen in trailer 200 sq. ft. Non-friable Minimal flooring Yellow and brown square pattern sheet vinyl flooring (homogenous 402)was sampled from underneath the carpeting. This material was found to contain 55% chrysotile asbestos. MATERIAL AREAS AMOUNT ASSESSMENT DAMAGE Yellow and Family room. 100 sq.ft. Non-friable Minimal brown square attern sheet C (I •n i floorin Brown adhesive(homogenous 406)was sampled from behind the shower vinyl walls in the master bathroom. I Non-friable I Minimal Brown and white hexagon pattern sheet vinyl flooring (homogenous 909)was sampled from the floor just inside the entry way to the trailer. The materials can be disposed of as general construction debris. LE%, fTATIONS OF SURVEY The sole purpose of this survey report is to document the asbestos containing materials(ACM) discovered in the survey and sampling of suspect materials by The Kriebel Company personnel, located at 8621 186`h Street Arlington,WA, on May 2,2001. In stnct accordance with NESHAPS regulations,this site visit consisted of a thorough visual walk through of the building for the purpose of viewing and sampling potential ACM. As asbestos surveys are non-comprehensive by nature,The Kriebel Company, Inc. cannot be held liable for materials which require destructive means to access, materials which are hidden from sight(e.g. materials hidden behind walls), materials which cannot be found due to their obscure nature, or which otherwise cannot be discovered with reasonable diligence.
