Dear Pastor White:
In review of the above noted project the following conditions of the conditional use permit
remain to be met:
1) Approval of the conditional use permit is subject to the applicant providing, prior to any
site construction, a site plan prepared by a licensed surveyor registered in the State of
Washington as a recordable survey indicating all parcels owned by the Assembly of God
Church and parties affiliated with same that have been included in this proposal. The
surveyor shall set monuments at street intersections, lot and block corners, boundary
angle points, points of curbs in streets, controlling corners on boundaries of the land and
at other points as required by the City Engineer. The type of monument and the method
of setting shall be specified by the City Engineer. The surveyor shall provide an
appropriate statement on the face of this document to certify that the work was prepared
by him or under his supervision, and that the record of survey is a true and correct
representation of the land surveyed. This record of survey is subject to the approval of
the City Engineer and shall be submitted along with the surveyors calculations for review
and approval. In addition this survey shall include information regarding the location and
dimensions of streets abutting this property, specifically Third Street and Stillaguamish
Ave. Should information indicate that additional right-of-way is needed to provide
adequate street widths for either of these streets, dedication shall be required by the
applicant. Dedication information shall be included within the record of survey
document.
2) Applicant is required to provide a waiver of protest for the formation of any local
improvement district that may benefit the property.
3) Park mitigation must be paid at the rate of.05C per square foot for the newly constructed
area.

I realize the survey should have been submitted prior to issuance of the building permit, but I
believe they may have the information and have just not submitted it. Our intention on the
survey was to get a starting point of record for this site in preparation for future requests. The
issue of the right of way is very important to use specifically on Third Street.

The center north-south beam line, as shown on the plans can be ommitted, provided the
following steps are taken:
1. 14-inch TJI/35X @ 19.2 inches on center are substituted for the 11-7/8" TH's shown
on the Foundation Plan.
2. (3) 2"x12" DF#2 are substituted for the 4"x10" east crawlspace beams. Nail-laminate
per structural notes.
3. 6"x6" HF#1 posts are substituted for the 4"x4" posts at the east crawlspace beam line
in the crawlspace.
The north/south 4"xl0" beams shown under the stairway walls on the Foundation Plan
are not required and should not have appeared on the approved plans.
The roof truss spacing can be increased to 24 inches on center.

Company Built-Up Roofing Systems Manual are applicable as part of
this specification.
Malarkey recommends the use of its High Performance Premium
Polyglass Cap Sheet No. 601 as a base flashing material. See
Section 5, Flashing Specifications, for details pertinent to the
requirements of the specific project.
Malarkey recommends the use of its SBS Mineral Walkboard No. 916
for all traffic areas.
4. 01 APPLICATION/INSTALLATION
A. Flashings: General
I. Shall be applied as per manufacturer's specifications. A
minimum of a 501 SBS Base Sheet and 601 Premium Gap Sheet
shall be used for all flashing membranes.
2 . Extend roofing membrane 2 inches or more above -all cants.
3 . All vertical surfaces shall be canted.

B. Vertical Flashings:
1. Shall. be minimum of 8 inches above roof membrane. (See

1. Prime both sides of all flanges to receive roofing with one
gallon of asphaltic primer per 100 square feet or roof
area and allow to dry thoroughly.
2 . Set metal in mastic and nail 4 inches on center to wood
nailers or insulation stops.
3 . All stacks shall have an 8 inch minimum height metal
flashing sleeve.
4 .02 PREPARATION & USE OF MATERIALS & EQUIPMENT
A. Asphalt Temperatures:
1. The asphalt roof coating may be heated with acceptable
heating equipment, no higher than 120 F. at holding tank.
4 .04 EXPANSION JOINTS
A. Contact Malarkey for specific application details.
4 .05 WATER CUT-OFFS
A. Divide roof area into areas as specified.
B. At the end of each day's work, provide temporary water cut-
offs at the edge of the completed roofing work. Remove cut-
offs when work commences.

C. Water cut-offs shall be one layer of No. 501 base sheet
embedded in a layer of asphalt.
4 . 07 ROSIN PAPER APPLICATION
A. Loosely tack rosin paper in place, using 2" side laps and 611
end laps. Apply over all wood nailing surfaces before
application of the base sheet.
4 . 08 BASE SHEET APPLICATION
A. Specified base sheet shall be mechanically fastened to deck
with Simplex type nails and caps, per requirements of Malarkey
Roofing. Side lap fasteners shall be 9" O.C. , placed within 2"
side lap. Fasteners in field shall be placed in two rows, with
each row 1311 in from the edge of the sheet, 18" C.C. , offset
9" from each other.
4 . 09 INTER PLY APPLICATION
A. Spray, squeegee or broom 2 1/2 gallon minimum of fluid
continous asphalt roof coating onto base sheet. Roll inter-
ply of 501 into coating, assuring adhesion with light brooming
if necessary. Avoid foot traffic immediately after
application. Ply sheets shall be shingled in, free of wrinkles
or f ish.mouths. Side laps shall be 211 . End laps shall be
minimum of 6 inches and offset a minimum of 12" inches apart.
B. Shall be lapped so water flows over or parallel to, but
never against the laps.
C. shall be installed specific to the system.
4 . 10 VALLEYS
A. Valleys shall receive an extra ply of No. 501 base sheet.
B. This extra ply shall be at least 36 inches wide and extend
at least 12 inches up the inclines of the valley.
C. This extra ply shall be laid prior to the application of the
roofing plies.
4 . 11 CAP SHEET APPLICATION
A. Cut 601 cap sheet into maximum 18 foot lengths. Allow to
relax minimum of one half-hour. Apply asphalt roof coating
onto back of cap sheet. Flop onto inner ply application with
2" side laps and minimum 611 end laps. Stagger end laps from
sheet to sheet minimum of three feet. Use loose ceramic
granules on asphalt bleed-out.

requirements should be shown and noted on the drawings for both toilets. This includes
5'-0" space, door clearance, grab rail location and accessory locations and hot water
protections. 17" to 19" is required tot he top of commode seat and 34" to 36" to top of
grab bar is required.
6. The guard railing at the "chair lift landing" should be not less than 42" high and have
openings through which no object 4" or greater can pass through 3-15/16" maximum
openings. Sec. 1712(a)
7. The space between the wood floor construction and the suspended ceiling below shall be
divided by draft stops installed so that the concealed space does not exceed 1000 SF and
so the horizontal dimension does not exceed 60 feet. Sec. 2516(f) 4A(iii)
8. The roofing material should be class "B" roof covering, the designer should submit the
manufacturer of the roofing with the UL listing and classification. Sec. 3203 and Table
No. 32-A.
9. The drawings should show how the ventilation of the attic is provided. sizes and location
of vents should be shown. Vent openings are required to have 1/a" mesh which reduces
the gross area by 25%. Sec. 3205(c). Attic ventilation should be computed at 1/150th
of the space ventilated.
10. Overflow drains having the same size as the roof drains should be provided. Roof
drawings should have independent drain lines. It is recommended that the overflow drain
lines discharge be "day lighted".
11. An occupant load sign should be posted in the upper floor classrooms having the folding
door. The occupant load is 61.
12. The upper floor corridor should have:
a. 1-hour fire resistant construction which should be shown on the drawings.
b. 20 minute labelled doors, frames and hardware.
C. Two exits from the corridor are required.
d. Exits from the corridor should be directly to the exterior or to a fire rated
corridor or to a rated stairway. Sec. 3305(a).

and hardware, including closures.
14. Doors should swing in the direction of exiting for item 13 above.
15. The lower floor existing corridor should be:

16. The landing in the stair on the stair side of the doors noted above should be not less than
44" in direction of travel.
17. Exit doors should be openable from the inside without the use of a key or special
knowledge. Manually operated edge or surface bolts are prohibited. Sec. 3304(c). Exit
doors should be not less than 3'-0" wide.
18. All stairs should comply with all sub sections of Sec. 3306, and should be detailed on
the drawings:
a. Riser heights and tread widths. 7" maximum riser, I I" min. tread.
b. 2" maximum round handrail required on each side.
C. 34" minimum to the top of handrail and not more than 38" maximum.
d. Enclosed space under stairs shall have 1-hour protection.
e. Open risers are not permitted.
f. Stair nosings shall be flush, slip-resistant and rounded to a radius of 1/2"
maximum. Risers shall be sloped or the underside of the nosings shall have an
angle of not less than 60 degrees from the horizontal. Nosings shall project nor
more than 11/2. inches.
g. The stairs should be 1-hour construction.
19. Assistive listening services shall be provided in the classrooms with the folding doors.
Sec. 3103a(2)B of the Washington State Building Code.

cedar. Heart cedar is not available so cedar is not approved. Sec.
"wood of Natural Resistance to Decay" and Sec. 2516 (c) 3.
13. The crawl space access opening should be not less than 18" X 24" .
Sec. 2516 (c)2. It now appears to go into the lift space.
14. The crawl space should have opening vents with a net area of not less than
one square foot for each 150 square feet. Screened vents should be provided
with 1/4" mesh screen. Sec. 2516(c)6. The screen reduces the area by about
25%,
15. The space between the wood floor construction and the suspended ceiling

16. Where post and beam construction in the crawl space is used, a positive
connection should be provided at the post/beam connection and to the post/footing
connection to ensure against uplift and lateral displacement. The connections
should be attached to both sides of the beam and the post with 2 - 16d nails
in the beam and 2 - 16d nails in the posts. Sec. 2516 (m) . The gusset should
be plywood or 1-1/2" thick lumber. A Post base should be provided.
17. All concrete walls should have not less than .0025% horiontal rebar and
.0015% vertical rebar, with the spacing not more than 18" o.c. , 8" concrete
walls should have No. 4 vertical rebar at 16" o.c. and No. 4 at 10" horizontally.
Sec. 2614, 2, 3 and 5. 6" walls require No. 4 vextical rebar at 18" and
No. 4 at 13" horizontal. This also applies to all foundation walls of non-
residential buildings. The structural notes should be corrected.
18. Exterior wall footings should be poured in undisturbed soil for a depth
of 711. Table 29-A and Footnote 2.
If the owner or contractor wants to your on undisturbed soil and fill after
pouring, the exterior fill adjacent to the exterior walls should be compacted
to 95% modified AASNO density and should be tested and inspected by a special
licensed inspector or a registered soils engineer.

There was additional discussion regarding traffic impact fees and water/sewer service fees.
Eric Wagner noted that the existing day care and nursery will not change with the addition. The
addition will serve the same number of people and will provide increased handicapped facilities
and accessibility.
There was no further public input. Mayor Kraskd closed the hearing and reopened the regular
meeting.
Cristy Brubaker, in response to question from the Council, reviewed the purpose of the
conditional use permit request.
Councilmember Tepfer suggested that the retaining wall be subject to SEPA review and only
become a part of the site plan after staff review determines that the wall is acceptable.
There was discussion regarding the retaining wall, site parking, attendance reports, probable
significant environmental impacts and status of the conditional use permit if the retaining wall
is deleted from the site plan.
It was moved by Councilmember Tepfer, seconded by Councilmember Randall, to approve the
conditional use permit as recommended by the Planning Commission with the deletion of the
retaining wall from the conceptual site plan.
There was some further discussion regarding the retaining wall. It was noted that any
construction would be required to meet all city codes and regulations.
The motion was voted upon. Councilmembers Tepfer and Randall voted aye. Councilmembers
Smith, Larsen, Caldwell, Hagenston and Regan voted no. The motion did not carry.
It was moved by Councilmember Hagenston, seconded by Councilmember Caldwell, to
approve the conditional use permit subject to the planning commission facts and findings,
mitigation agreement and that any future discussion of the retaining wall be with the
planning commission and gngineering department; and include a soils analysis and
engineering report. All councilmembers voted aye except Randall. Motion carried.
A five minute recess was called after which the meeting reconvened.

RE: Geotechnical recommendations for proposed rockery adjacent
to north parking area expansion.
Dear Reverend White,
Congratulations on your newly remodeled church and anticipated
parking expansion. I 'm pleased to submit this report with
respect to the existing soil conditions adjacent to the proposed
rockery within the new parking area. .
Surface and subsurface soil conditions were inspected during
a site visit on 6/23/1994 . Shallow hand explorations generally
encountered a loose gravely top soil 6-8" deep, unlain with
very dense,silty, glacial till. In addition visual inspection
revealed the sloped area to be well consolidated which confirmed
the findings of the 5/4/1994 soil report by Sundin Soil
Services .
The integrity of the slope behind the proposed rockery is
historically very stable. According to our discussion the slope
has experienced very little erosion within the past 20 years
and remains basically unchanged.
No ground water seepage was observed from the exposed slope
surface. Only minor surface run-off is anticipated from the
grassed areas above.
CONCLUSIONS AND RECOMMENDATIONS
It is my opinion with respect to the north parking area
expansion, that construction of a rockery at a nominal height
of 8 ' or less, is feasible given the existing soil conditions
as described. Rockeries greater than 8 ' will require a separate
analysis, and are not recommended.
Rockeries should be installed per standards accepted by the
Association of Rockery Contractors, and local, state, and
national safety regulations.

Based upon my observations, the site slopes are stable and should
remain so, provided the following recommendations are
implemented:
1 ) Cuts should not exceed 8 ' height.
2 ) Installation of rockeries should be done during dry
weather, if possible.
3 ) Disturbance of the slope should be minimized to reduce
potential slope erosion and movement.
Damaged vegetation should be immediately replanted and
maintained.
4 ) Excavated soils from slopes should not be reused for
rockery backfill.
Backfill is to be a free-draining structural fill
containing no organics. The fill should contain no
more than ( five) 5% silt or clay particles and have
no particles greater than 4" .
5 ) A 4" rigid PVC footing drain shall be placed behind
the rockery and surrounded by at least ( 6) six inches
of 1 " minus washed rock wrapped in non-woven geotextile
filter fabric. The drainline will be connected into
the existing storm system.
6) Surface water should be directed away from the top of
the slope where possible.
LIMITATIONS
This analysis is based upon site conditions at the time of
the site visit done 6/24/1994,with the assumption that soils
observed are representative of subsurface conditions for the
entire slope area. If conditions encountered during excavation
vary significantly from those described , I should be contacted
immediately to reconsider recommendations as necessary. Soil
conditions can vary significantly even in localized areas and
often aren' t revealed until excavation.

Re: Soils Study for slope stability analysis for north parking
lot area expansion and rockery.
Dear Pastor White,
I have a BS in Civil Engineering and a certified EIT certificate.
I work work with various Civil and Geotecnical engineers in the
area and conduct soil analysis for septic system designs on a
weekly basis.
I conducted a side slope soil analysis on the exposed excavated
existing slope on the east side of the church's north parking
lot. Two sites on the face of the excavated soil were visually
and texturally inspected . Both sites appeared to have almost
identical characteristics which follow:
0-6' Brown Cob. Gray. Silt Loam. Soil is lightly compacted
and transmits water very poorly. Soil structure is massive
and only slight erosion was exhibited on the surface after
apparently many years of exposure. The soil is a United
Soils Classification System Type "B" moderately cohesive
soilwith no visible fissures.

compacted and nearly impereable. Soil structure is massive
and only slight erosion was exhibited on the surface after
