The following utility items are required to be completed before the city will provide water and sewer service to the site. 1 . A Double Check Valve Assembly (DCVA) shall be installed on the downstream side of the water service meter. After the DCVA has been installed, you:will = need to contact our. office to arrange for an inspection. Once we: have inspected and approved the installation, you will be required'to have a.state certified Backflow Assembly Tester test- the DCVA to assure its: proper operation. A copy of the test report shall be submitted to our office for review and approval: A copy of the installation specifications for a DCVA is attached for your use. 2. A sewer stub shall be extended--from the sewer main to the property at your expense.'You will need to hire a licensed and bonded contractor to perform the work. After the sewer stub construction is completed, an appropriate road patch shall be made,, and any sidewalk, curb, gutter etc. that has been cut through shall be appropriately repaired. With the addition of the following, the plans are approved. 1 . All food service equipment must be listed by the National Sanitation Foundation (NSF) for its intended use. 2. Plumbing must meet state and local codes. 3. A mop sink or utility sink is required. 4. Cabinetry in the food preparation areas may be constructed using exterior grade plywood AB grade or better. Particle board is not acceptable. 5. Extra wall protection is required on walls behind sinks and food preparation tables. A 16 inch high backsplash of plastic laminate, fiberglass reinforce plastic or equal is acceptable. Plastic coated hardboard is not acceptable. 6. The ceiling above the food preparation areas must be nonabsorbent, smooth, and easily cleanable. 7.
