To receive approval for occupancy from the Water Department, you will need to complete the items fisted below. Cross Connection 1. Install a State of Washington approved, non-removable Atmospheric Vacuum Breaker(AVB)on the hose connection end of the yard hydrant. 2. Your construction drawings indicate that a Double Check Valve Assembly (DCVA) will be installed on the water supply line to the new emergency eyewash. The DCVA does not need to be installed on the emergency eyewash supply line; it has no functional use in the proposed location and provides no protection to users of the emergency eyewash. The Water Department is not requiring the DCVA to be installed, however you should check with David Anderson, City of Arlington Building Official, 360-403-3432 to make sure this is not a requirement of the Uniform Plumbing Code. 3. Call for inspections at the following points of construction. a. Prior to covering interior piping. b. Prior to covering underground piping. c. Prior to requesting final inspection by the Building Department. I can be reached at 360-403-3505 to answer any questions you may have. 0 Page i City of Arlington Building Dept PUBLIC WORKS CHECKLIST PERMIT# Vt '%4 DATE LEGAL Plat Lot Tax ID# NAME t` (,` , ` I ' , ADDRESS C YJ Y < Avc_ BUILDING USE ?t!.L_ (,r; L (.D(1) # of BUILDING UNITS f Existing Required Signature Date Water Meter IZ Fire Hydrant Side Sewer Permit L/ Monitoring Manhole / Cross-Connection Control 1/ Sewer: Off site On site t/ Water: Off site On site Pretreatment Discharge Permit Water/Sewer Fees Date received Q Z E C E I V E D Date Yellow returned <O ppu MAY 2 8 2002 Date Pink returned Utilities Div.
