Complete and return the enclosed Cross Connection Survey and Industrial/Commercial Waste Discharge Permit application. 2. A Washington State approved Reduced Pressure Backflow Assembly (RPBA) must be installed on the downstream side of the water meter, per City standard W-16. The RPBA shall be installed immediately on the downstream side of the water meter, before the first point of use or branch connection. Alternatively, the RPBA may be installed inside the building in a mechanical/equipment room, where proper drainage and space can be provided. If the RPBA is installed indoors, it must be installed before the first point of use or branch connection. An RPBA has different requirements than the DCVA that has been required in the past; please read through the attached standard W-16 and familiarize yourself with the differences. 3. After the RPBA is installed, it must be tested by a Washington State certified Backflow Assembly Tester, at the owner's expense, then be inspected by the Water Department. Please provide a minimum 24-hour notice when calling for inspection. 4. It is recommended that you install an expansion tank on the water system to allow for thermal expansion. 5. What size of water meter is required? RECEIVED 6. Will you be installing a fire sprinkler system or a lawn/landscape irrigation sytelr1 5 2001 7. Inspection of water system piping is required at the following times: N a. Before underground piping is covered. UTY OF AaLiiVG i ON b. Before interior piping is covered. 0 Page 1 8. A walk-through inspection of your facility is required priorto requesting final inspection from the Building Department. Please provide a minimum 24-hour notice when calling for inspection.
