While on-site Coast provided video documentation of epoxy activities for the west half of the
C/12 footing, completed on Saturday, 1-12-19. The contractor appeared to install anchors
correctly and in accordance with SDA letter 8163. However, the adhesive used was Sika Pro
Select epoxy, with an expiration date of 7-25-19. Acceptance of the use of this epoxy in place
of Simpson Set-XP from the engineer is required for conformance.

Comments: GeoTest was on site as requested to provide compaction testing and observation
of asphalt placement on 63rd Ave. All locations are approximate. A 4" lift of 3/4" ATB was
placed and compacted using a large vibratory roller. A total of 410 tons of ATB was placed on
this date. Compaction at test locations met project requirements.

Comments: GeoTest was on site as requested to provide hot mix asphalt compaction testing
and placement observation on the 63rd Ave roadway. All locations are approximate. A 3"
wearing lift of 1/2" HMA was placed and compacted using dual drum vibratory rollers. A total of
316.63 tons was placed on this date. Compaction at test locations met requirements
established by the approved plans.

Comments: GeoTest on-site for inspection of the slab on grade located at grids B-D/8-11.3 and
C-D/11.3-14. Performed final inspection of steel, which was in conformance with structural
drawings except that hairpins at AB's did not have adequate coverage at the outer edge of
forms. Concrete was placed by pump and consolidated through mechanical vibration, and
mechanical screed.
*The Stanwood Redi-Mix QC provided the thermometer for mix temperature readings.
AMENDED 3/1/19: Error identified in Mix desc.; updated for accuracy.

at grids B-D/2-8.5. Reinforcing steel was in conformance with structural drawings. However the
hairpins for the columns around the perimeter of the slab did not meet concrete cover
requirements. Notified the contractor of the deficiency. Please see figure 2 below for reference

1-33. Consolidation of the concrete was achieved by mechanical vibration. It was noted that
panels 10, 18, and 31 had fixes applied to the embeds in accordance with drawing 3/3 of the
Plinth Repair Drawings. Also, under engineer advisement, the contractor separated the rebar

Upon GeoTest's arrival, five locations had been selected and potholed by the contractor to 2-3'
from the adjacent grades. Two locations were potholed on both the west and east sides of the
loading ramp retaining walls, with one test located north of the loading ramp. Tests 1 through 5
were taken at the base of the potholed locations. Additionally, tests were taken after backfilling
those locations (tests 6 through 10). One sample of the material was taken beneath the gauge
at test location #1 for a sieve/proctor laboratory test.
After initial density testing at the base of the potholed locations, the areas were backfilled. The
material was placed in 3 approximately equal lifts and compacted with a CAT excavator with a
mounted hoe-pack. The top surface was also compacted with a single drum vibratory roller
after the hoe-pack was completed. During placement and compaction, water was added to the
material by a hose.

While onsite, proof rolls were performed on the subgrade on either side of the ramps and on
the overhead doors on the north/east sides of the building using a loaded dump truck. The
subgrade was observed to be firm and unyielding. Some yielding was observed over the
infiltration trench at the large overhead door on the east side of the building. This material
consisted of drain rock - it's our understanding that some drain rock will be removed and
replaced with crushed rock. The GeoTechnical Engineer was onsite and observed the proof
rolls of the referenced areas.
It is our understanding that the contractor will perform additional compactive effort at a later
date next to the building. GeoTest will return when requested.

Previously inspected items and items noted in FOR memo dated 3/8/19
Comments: GeoTest was on-site to review the as-built condition of the completed foundation connection points
and review available information for the revised foundation connections at 13/2, D/14 and D/2. In response to
the SDA letter provided to us dated 3/8/19 we can provide the following clarifications and verification:
In regards to the first bullet point noted in SDA letter referenced above,we have amended our AA003 report
to reflect conformance of this item.
In regards to the 2nd bullet point,we can confirm that the reinforcement configuration and concrete were in
conformance with the revised"plinth repair"drawings dated 1/24/19 as stated in our report CB011.Welding
inspection was not conducted prior to concrete placement at these locations.We have been provided with
applicable welder's certification, photos of the welded rebar to plate, and material sales report for the
A706 rebar supplied. Review of these items does not constitute special inspection, but the provided
information suggests that the welder was certified to complete the work, the A706 rebar supplied meets the
grade requirements for welded rebar, and that the bars were welded to the plate with SMAW process in lieu of
automatic stud welding.
In regards to the third bullet point noted in SDA letter referenced above,we have independently verified
each of the noted in-progress or discrepant items noted in IR005 and all were corrected per plan prior to
concrete placement. The only exception being the first item noted in IR005, lack of coverage over hairpin
ties. This specific item was noted in discrepant reports CB007 and C13008.

While on site we reviewed the memoranum by Structural Design Associations, dated April 1,
2019, titled "Panel Deficiencies".
Cracks in Panels:
Per SDA's letter referenced above (2.E) - Panel cracks that are located more than 4" from the
weld shall be reported to SDA in writing (reference sketch titled "Cracks at Foundation/Panel
Clip"). GeoTest noted the following:
• All exterior foundation/panel connections were measured from the top of the crack to the
weld. Six locations exceeded 4" and are detailed on subsequent pages including three photos
of each location. See figures 1 through 18.
Foundation/Panel Clip Inset:
SDA's letter, item 3, notes that at least one panel/foundation clip is inset into the panel. The
letter states that, "the site condition is deemed to be structurally adequate if the inset is 2" or
less from the exterior face of the panel." GeoTest noted the following:
• All exterior foundation/panel connection clips were generally flush with the exterior face of the
panel except one location which was inset. The inset was measured and found to be under the
2" criteria. See figures 19 and 20.
Contractor was made aware of our findings while on site.

GeoTest was on site as requested to inspect the repair of the crack damage of concrete at the
panel embed weld plates.
The buildings bottom plates were coated with Armatec-110, bonding primer and corrosion
inhibitor. Two coats were applied using a stiff brush (see photo) and the manufacturer's
instructions were followed. The substrate prep, the mixing of the product, the application of the
product and curing time and conditions all were as recommended by the manufacturer.
At some locations, concrete appeared to have been saw cut to a depth of 1/2" around the plate
and bushed hammered within the saw cut area to the same 1/2" depth. At these locations, the
contractor applied Sika 123 Plus repair mortar in addition to the Armatec-110. The
manufacturer's instructions were again followed with the application of this product.

GeoTest was on site as requested to inspect the concrete repair of the tilt-up panels. The
cracks were just above the bottom embed plates at six panels on the buildings north side.
The contractor followed the "Panel Deficiencies" report (1st amendment) dated 5/23/19 while
conducting the repairs.
Location of the repairs: Line B, six locations, all were found at the bottom of the concrete
panels and just off an embed plate.
The cracks were cleaned using compressed air removing any loose material and concrete dust
prior to conducting the repair. See photo below.
The contractor installed injection ports over each crack spaced at approximately two inches on
center. The ports were installed and the surface of the crack was sealed using Sikadur-31
Hi-Mod Gel.
After the Hi-Mod Gel cured the contractor pressure injected the cracks using the ports (See
photo) with Sikadur -35 Hi-Mod (low viscosity) epoxy.

GeoTest was on-site to review the installation of Sika Sikalastic 320 NS installed along the
foundation wall and footing along the building perimeter, Gridlines B/2-11, 2/13-D, D/2-14, and
14/C-D.
The material was installed to an average 40-45 mil thickness and was installed without any
observable voids, bubbling, pinholes or other gaps in coverage. Material installed extended
from grade line down the foundation wall, over the top of the footing, and approximately 2-3"
down the face of the footing. The material was installed in a monolithic, unbroken manner,
except at entryways where it was not installed to accommodate pending pour-backs.
Material application thickness was not specified, and the extents of the installation were not
clearly defined in project documents, but work installed to date appeared consistent with
manufacturer requirements.

OBSERVATION OF:Proof Roll Conducted on CSBC Material for Future Paving
GeoTest was on site as requested to observe a proof roll at the future location of 63rd Ave,
located on the east side of the project. The subgrade consisted of native sandy soils overlaid
by approximately 6 inches of import CSBC.
GeoTest witnessed Coast perform a proof roll utilizing a dump truck fully loaded with fill
material. There were no signs of yielding or rutting during the proof roll on this date. The
subgrade condition was found to be firm and unyielding. Based on the results of the proof roll,
it's our opinion that the subgrade has been properly
prepared.
Coast was made aware of GeoTest's results.

were checked using a calibrated torque wrench to determine if they were tightened to the
predetermined torque values listed above.
