Arlington,Washington INTRODUCTION An inspection for the presence of asbestos containing building was conducted on June 15,2012, in the house and shed located at 420 Alcazar Street in Arlington, WA.This inspection is to identify asbestos containing materials that will affect the proposed demolition of the structures. Jennifer from Hanson&Hanson Construction requested the asbestos inspection. Sara Kriebel, an AHERA certified building inspector from The Kriebel Company,Inc., conducted this asbestos inspection. Samples were analyzed by NVL Laboratories,Inc.,which is a NVLAP certified laboratory. The following report contains material descriptions, sample locations, assessment of asbestos containing building materials, laboratory results and accreditation, and inspector certification. AHERA Building Inspector: Sara Knebel Inspector Accreditation Number: 135578 Expiration Date: February 28, 2013 Signa r- - i LIMITATIONS OF SURVEY The sole purpose of this inspection report is to document the asbestos containing materials discovered during the inspection and sampling of suspect materials by The Knebel Company, in the house and shed located at 420 Alcazar Street in Arlington, WA. In strict accordance with NES14APS regulations, this site visit consisted of a thorough visual walk through of the buildings for the purpose of viewing and sampling potential asbestos containing materials. As inspections are non-comprehensive by nature,The Knebel Company, Inc. cannot be held liable for materials which require destructive means to access, materials which are hidden from sight(e.g. materials hidden behind walls),materials which cannot be found due to their obscure nature, or which otherwise cannot be discovered with reasonable diligence. Due to electrical shock hazard, none of the wiring was sampled. All sample analysis reported here-in were analyzed using analysis method specified in Appendix E, subpart E,40 CFR part 763, section 1,Polarized Light Microscopy. This survey does not include documentation for other hazardous materials that may exist on site, including: lead,PCB,soil contamination due to underground storage tanks, discarded oils, paints or chemicals, etc. WAC 296-62-07703 Definitions—Asbestos-containing materials (ACM)means any material containing more than 1% asbestos. l f SECTION 2 INSPECTION SUMMARY ASBESTOS SUMMARY 420 Alcazar Street,Arlin0on,WA. Laboratory ; Managernent Trafning 9122 169th Ave. NE Granite Falls, WA 98252 RE: Bulk Asbestos Fiber Analysis,NVL Batch#1209200.00 Dear Ms. Knebel, Enclosed please find test results for the bulk samples submitted to our laboratory for analysis. Examination of these samples was conducted for the presence of identifiable asbestos fibers using polarized light microscopy (PLM) with dispersion staining in accordance with U.S. EPA/600/M4-82-020 Test Method. For samples containing more than one separable layer of materials, the report will include findings for each layer (labeled Layer 1 and Layer 2, etc. for each individual layer). The asbestos concentration in the sample is determined by visual estimation. For those samples with asbestos concentrations between 1 and IQ percent based on visual estimation, the EPA recommends a procedure known as point counting (NESNAPS, 40 CFR Part 61). Point counting is a statistically more accurate means of quantification for samples with low concentrations of asbestos. If you would like us to further refine the concentration estimates of asbestos in these samples using point counting, please let me know. This report is considered highly confidential and will not be released without your approval. Samples are archived for two weeks following analysis. Samples that are not retrieved by the client are discarded after two weeks. Thank you for using our laboratory services. Please do not hesitate to call if there is anything further we can assist you with.
