(PBS) performed a limited hazardous materials survey of the single- family home,garage,and shed located at 18506 Smokey Pt. Boulevard,Arlington,Washington in conjunction with the planned demolition. The intent of this investigation is to ensure that AVS Communities is in compliance with applicable regulatory requirements that a "good faith inspection"for ACMs be performed prior to demolition activities. At the request of AVS Communities,all accessible areas associated with the project were inspected for the presence of asbestos-containing materials (ACMs),lead-containing paint(LCP), PCB-containing light ballasts and mercury-containing fluorescent lamps. 1.2 Building Description PBS'survey of the noted property encompassed three (3)structures:The main residence,the unattached garage,and the shed: The Main Residence; is a wood-framed single-story structure with a finished attic and an unfinished basement. The interior finishes with the main residence generally include tongue and groove wood floor planking covered by a combination of vinyl tile,sheet vinyl, carpeting, or wood laminate flooring.The walls are textured gypsum wallboard except for the bathroom which has ceramic tile on the gypsum wallboard.There is fiberglass batt insulation within the wall cavities.The ceilings are typically textured gypsum wallboard throughout the main floor and the attic.At the time of PBS' survey, most of the exterior wood siding, copper plumbing, and all of the main floor aluminum-framed windows had been removed.The perimeter attic spaces surrounding the central finished attic portion have fiberglass and blown-in insulation.The sub-grade unfinished basement is on a concrete slab while other crawlspace areas below the main floor consist of dirt (vapor barrier not observed) and fiberglass batting directly attached to the underside of the main floor planking.The roofing of the main residence consists of asphalt shingles with a vapor barrier on plywood sheathing. The Garage is slab on grade wood-framed unfinished structure with a reinforced plastic/vinyl roof covering. There is a wood-framed overhang off the west side of the garage that has an asphaltic shingle roof The Shed is a wood-framed unfinished structure with wood flooring and an asphalt shingle roof. 1.3 Survey Process Accessible areas included in the project scope were inspected by AHERA Certified Building Inspectors Mike Smith (Cert. No.IR-18-6154B Exp. 03/8/2019) and Justin H. Day(Cert. No.IR-18-6181B Exp.04/25/2019) on October 10, 2018. PBS endeavored to inspect all accessible areas included in the scope of work as outlined above. Inaccessible areas consist of those requiring selective demolition,fall protection, or confined space entry protocols in order to gain access. When observed, suspect materials were sampled.All samples were assigned a unique identification number and transmitted for analysis to NVL Laboratory(NVLAP#201057-0) under chain-of-custody protocols. Samples were analyzed according to EPA Method 60OR-93/116 using Polarized Light Microscopy(PLM),which has a reliable limit of quantification of 1%asbestos by volume.Information regarding the type and location of sampled materials can be found on the attached PLM Sample Inventory. PBS endeavored to determine the presence and estimate the condition of suspect materials in all accessible areas included in the scope of work.While PBS has endeavored to identify the ACM that may be found in concealed locations, additional unidentified ACM may exist. 2 FINDINGS 2.1 Asbestos-Containing Materials(ACMs) Federal and state regulations define an asbestos-containing material as any material that contains greater than 1%asbestos. PBS collected twenty-nine(29) bulk samples of suspect ACMs. • Red Vinyl Floor Tile—below laminate and sheet vinyl flooring in kitchen and dining room (approx. 400 SF); • Asphaltic Shingles—back side of garage overhang (approx.400 SF). The following materials were sampled and found not to contain detectable concentrations of asbestos: o Blown-in attic insulation—exposed attic spaces o Orange peel type texture—all interior walls • Gypsum wall board and joint compound—throughout building o Yellow backsplash mastic—kitchen o Ceramic tile,grout and mastic—bathroom o Asphaltic adhesive tape and yellow mastic—Behind mirrors above bathtub in restroom o White paint and texture overspray—on wood under laminate flooring and sheet vinyl flooring o Black vapor barrier—Under wood,tongue and groove flooring—Bedrooms o Red vinyl floor tile and Tar paper—below laminate,sheet vinyl flooring and ACM red vinyl floor tile in kitchen and dining room o Beige sheet vinyl flooring—under laminate and exposed in kitchen, dining room and east entry way o Tarpaper vapor barrier—exterior exposed and under siding o Asphaltic shingles and vapor barrier—roof o Reinforced vinyl roofing—garage o Asphaltic shingle—south shed For a complete listing of representative bulk sampling and associated laboratory analysis, refer to the Attachments. 2.2 Lead-Containing Components Nine (9) representative painted coatings were sampled for lead content. The samples were assigned unique identification numbers and transmitted to NVL Laboratory(NVLAP#201057-0) in Seattle,Washington under chain-of-custody protocols for analysis using Flame Atomic Absorption. Lead was detected in the following painted coatings. • Dark blue paint on exterior vertical and horizontal wood trim (garage)—9.7% lead • Grey paint on exterior siding (garage)—0.11%lead • White paint on the wood tongue and groove flooring (below other flooring layers—residence)— 0.029% Refer to the attachments for an inventory of paint samples collected and associated laboratory analysis. 18 PBS Project No.401391.October 16, 0 2 PBS Pro 00202 Limited Hazardous Materials Survey Report Villas at Arlington—18506 Smokey Pt.Boulevard AVS Communities Arlington,Washington 2.3 Mercury-Containing Components All fluorescent light tubes and compact fluorescent bulbs are presumed to contain mercury. Approximately twelve (12) compact, eighteen (18) 4', Six (6) 2' U-shaped, and eight (8) 1' fluorescent bulbs were observed in various locations throughout the noted surveyed structures. 2.4 PCB-Containing Components PBS found all light ballasts throughout the surveyed structures to be of the magnetic type. Magnetic ballasts are considered as PCB-containing regardless of labeling.There are approximately twelve(12) ballasts throughout the structures. PBS also noted a 1-gallon tin can on the basement storage shelving with approximately eight(8) round ballasts/capacitors within.These ballasts/capacitors should be considered PCB- containing as well. 3 RECOMMENDATIONS 3.1 ACMs PBS recommends that all ACMs be removed prior to demolition activities by a qualified Washington State licensed asbestos abatement contractor according to applicable local, state and federal regulations. The possibility exist that suspect ACM may be present at concealed locations in wall and ceiling cavities, within HVAC equipment and potentially in other select concealed areas. These may include, but are not limited to waterproofing membrane,vapor barriers, internal gasketing, mastics, caulking,and sealants on HVAC equipment, construction adhesives, electrical insulators, below grade pipe covering and insulation. In the event that suspect ACMs not included in this report are encountered during construction, contractors should stop work immediately and inform the Owner promptly for confirmation testing. All untested materials should be presumed asbestos-containing or tested for asbestos content prior to impact. 3.2 Lead Containing Paint Representative painted coatings were found to contain lead by laboratory analysis. Impact of painted surfaces with detectable concentrations of lead requires construction activities to be performed according to Washington Labor and Industries regulations for Lead in Construction (WAC 296-62-155).Workers impacting LCP should be provided the proper personal protective equipment and use proper work methods to limit occupational and environmental exposure to lead until an initial exposure assessment has been conducted. Painted coatings may exist in inaccessible areas of the work area or in secondary coatings.Any previously unidentified painted coatings should be considered lead containing until sampled and proven otherwise. 3.3 Mercury-Containing Components Fluorescent lamps are known to contain mercury and mercury vapors. All fluorescent lamps at this site are presumed to be mercury-containing. PBS recommends that all fluorescent lamps be carefully handled and recycled/disposed of in accordance with the contract documents and applicable regulations during demolition activities. Breakage of lamps should be avoided to prevent potential exposures to mercury.Washington Department of Safety and Health requires specific training, handling, engineering controls and disposal practices when performing this work.All waste shall be handled in accordance with WAC 173-303. Magnetic ballasts and stored ballasts/capacitors should be presumed to contain PCBs and properly removed, stored, transported and disposed of in accordance with Washington Administrative Code (WAC) 173-303 Dangerous Waste Regulations and 40 CFR Part 761 Subpart D.Electronic ballasts do not contain PCB's and can be disposed of as general debris in compliance with applicable codes and endpoint facility requirements. Boulevard AVS Communities Arlington,Washington 1 INTRODUCTION 1.1 Project Background PBS Engineering and Environmental, Inc. (PBS) performed a limited hazardous materials survey of the single- family home, garage, and shed located at 18506 Smokey Pt. Boulevard, Arlington, Washington in conjunction with the planned demolition. The intent of this investigation is to ensure that AVS Communities is in compliance with applicable regulatory requirements that a "good faith inspection" for ACMs be performed prior to demolition activities. At the request of AVS Communities, all accessible areas associated with the project were inspected for the presence of asbestos-containing materials (ACMs), lead-containing paint (LCP), PCB-containing light ballasts and mercury-containing fluorescent lamps. 1.2 Building Description PBS' survey of the noted property encompassed three (3) structures: The main residence, the unattached garage, and the shed: The Main Residence; is a wood-framed single-story structure with a finished attic and an unfinished basement. The interior finishes with the main residence generally include tongue and groove wood floor planking covered by a combination of vinyl tile, sheet vinyl, carpeting, or wood laminate flooring. The walls are textured gypsum wallboard except for the bathroom which has ceramic tile on the gypsum wallboard. There is fiberglass batt insulation within the wall cavities.The ceilings are typically textured gypsum wallboard throughout the main floor and the attic. At the time of PBS' survey, most of the exterior wood siding, copper plumbing, and all of the main floor aluminum-framed windows had been removed. The perimeter attic spaces surrounding the central finished attic portion have fiberglass and blown-in insulation.The sub-grade unfinished basement is on a concrete slab while other crawlspace areas below the main floor consist of dirt (vapor barrier not observed) and fiberglass batting directly attached to the underside of the main floor planking. The roofing of the main residence consists of asphalt shingles with a vapor barrier on plywood sheathing. The Garage is slab on grade wood-framed unfinished structure with a reinforced plastic/vinyl roof covering. There is a wood-framed overhang off the west side of the garage that has an asphaltic shingle roof The Shed is a wood-framed unfinished structure with wood flooring and an asphalt shingle roof. 1.3 Survey Process Accessible areas included in the project scope were inspected by AHERA Certified Building Inspectors Mike Smith (Cert. No. IR-18-6154B Exp. 03/8/2019) and Justin H. Day (Cert. No. IR-18-6181B Exp. 04/25/2019) on October 10, 2018. PBS endeavored to inspect all accessible areas included in the scope of work as outlined above. Inaccessible areas consist of those requiring selective demolition, fall protection, or confined space entry protocols in order to gain access. When observed, suspect materials were sampled. All samples were assigned a unique identification number and transmitted for analysis to NVL Laboratory (NVLAP#201057-0) under chain-of-custody protocols. Samples were analyzed according to EPA Method 60OR-93/116 using Polarized Light Microscopy (PLM), which has a reliable limit of quantification of 1% asbestos by volume. PBS endeavored to determine the presence and estimate the condition of suspect materials in all accessible areas included in the scope of work.While PBS has endeavored to identify the ACM that may be found in concealed locations, additional unidentified ACM may exist. 2 FINDINGS 2.1 Asbestos-Containing Materials (ACMs) Federal and state regulations define an asbestos-containing material as any material that contains greater than 1% asbestos. PBS collected twenty-nine (29) bulk samples of suspect ACMs. • Red Vinyl Floor Tile— below laminate and sheet vinyl flooring in kitchen and dining room (approx. 400 SF); • Asphaltic Shingles— back side of garage overhang (approx.400 SF). The following materials were sampled and found not to contain detectable concentrations of asbestos: o Blown-in attic insulation—exposed attic spaces o Orange peel type texture—all interior walls o Gypsum wall board and joint compound —throughout building o Yellow backsplash mastic— kitchen o Ceramic tile, grout and mastic— bathroom o Asphaltic adhesive tape and yellow mastic— Behind mirrors above bathtub in restroom o White paint and texture overspray—on wood under laminate flooring and sheet vinyl flooring o Black vapor barrier— Under wood, tongue and groove flooring — Bedrooms o Red vinyl floor tile and Tar paper— below laminate, sheet vinyl flooring and ACM red vinyl floor tile in kitchen and dining room o Beige sheet vinyl flooring— under laminate and exposed in kitchen, dining room and east entry way o Tarpaper vapor barrier—exterior exposed and under siding o Asphaltic shingles and vapor barrier— roof o Reinforced vinyl roofing —garage o Asphaltic shingle—south shed For a complete listing of representative bulk sampling and associated laboratory analysis, refer to the Attachments. 2.2 Lead-Containing Components Nine (9) representative painted coatings were sampled for lead content. The samples were assigned unique identification numbers and transmitted to NVL Laboratory (NVLAP#201057-0) in Seattle, Washington under chain-of-custody protocols for analysis using Flame Atomic Absorption. Lead was detected in the following painted coatings. • Dark blue paint on exterior vertical and horizontal wood trim (garage)—9.7% lead • Grey paint on exterior siding (garage) —0.11% lead • White paint on the wood tongue and groove flooring (below other flooring layers— residence)— 0.029% Refer to the attachments for an inventory of paint samples collected and associated laboratory analysis. Boulevard AVS Communities Arlington,Washington 2.3 Mercury-Containing Components All fluorescent light tubes and compact fluorescent bulbs are presumed to contain mercury. Approximately twelve (12) compact, eighteen (18) 4', Six (6) 2' U-shaped, and eight (8) 1' fluorescent bulbs were observed in various locations throughout the noted surveyed structures. 2.4 PCB-Containing Components PBS found all light ballasts throughout the surveyed structures to be of the magnetic type. Magnetic ballasts are considered as PCB-containing regardless of labeling. There are approximately twelve (12) ballasts throughout the structures. PBS also noted a 1-gallon tin can on the basement storage shelving with approximately eight (8) round ballasts/capacitors within.These ballasts/capacitors should be considered PCB- containing as well. 3 RECOMMENDATIONS 3.1 ACMs PBS recommends that all ACMs be removed prior to demolition activities by a qualified Washington State licensed asbestos abatement contractor according to applicable local, state and federal regulations. The possibility exist that suspect ACM may be present at concealed locations in wall and ceiling cavities, within HVAC equipment and potentially in other select concealed areas. These may include, but are not limited to waterproofing membrane, vapor barriers, internal gasketing, mastics, caulking, and sealants on HVAC equipment, construction adhesives, electrical insulators, below grade pipe covering and insulation. In the event that suspect ACMs not included in this report are encountered during construction, contractors should stop work immediately and inform the Owner promptly for confirmation testing. All untested materials should be presumed asbestos-containing or tested for asbestos content prior to impact. 3.2 Lead Containing Paint Representative painted coatings were found to contain lead by laboratory analysis. Impact of painted surfaces with detectable concentrations of lead requires construction activities to be performed according to Washington Labor and Industries regulations for Lead in Construction (WAC 296-62-155).Workers impacting LCP should be provided the proper personal protective equipment and use proper work methods to limit occupational and environmental exposure to lead until an initial exposure assessment has been conducted. Painted coatings may exist in inaccessible areas of the work area or in secondary coatings. Any previously unidentified painted coatings should be considered lead containing until sampled and proven otherwise. 3.3 Mercury-Containing Components Fluorescent lamps are known to contain mercury and mercury vapors. All fluorescent lamps at this site are presumed to be mercury-containing. PBS recommends that all fluorescent lamps be carefully handled and recycled/disposed of in accordance with the contract documents and applicable regulations during demolition activities. Breakage of lamps should be avoided to prevent potential exposures to mercury.Washington Department of Safety and Health requires specific training, handling, engineering controls and disposal practices when performing this work.All waste shall be handled in accordance with WAC 173-303. Magnetic ballasts and stored ballasts/capacitors should be presumed to contain PCBs and properly removed, stored, transported and disposed of in accordance with Washington Administrative Code (WAC) 173-303 Dangerous Waste Regulations and 40 CFR Part 761 Subpart D. Electronic ballasts do not contain PCB's and can be disposed of as general debris in compliance with applicable codes and endpoint facility requirements. Preparation of these samples was conducted following protocol outlined in EPA Method SW 846 -3051 unless stated otherwise. Analysis of these samples was performed using analytical instruments in accordance with U.S. EPA, NIOSH, OSHA and other ASTM methods. For matrix materials submitted as paint, dust wipe, soil or TCLP samples, analysis for the presence of total metals is conducted using published U.S. EPA Methods. Paint and soil results are usually expressed in mg/Kg which is equivalent to parts per million (ppm). Lead (Pb) in paint is usually expressed in mg/Kg (ppm) , Percent(%) or mg/cm2 by area. Dust wipe sample results are usually expressed in ug/wipe and ug/ft2. TCLP samples are reported in mg/L (ppm). For air filter samples, analyses are conducted using NIOSH and OSHA Methods. Results are expressed in ug/filter and ug/m3. Other matrix materials are analyzed accordingly using published methods or specified by client. The reported test results pertain only to items tested and are not blank corrected. For recent regulation updates pertaining to current regulatory levels or permissible exposure levels, please call your local regulatory agencies for more details. This report is considered highly confidential and will not be released without your approval. Samples are archived for two weeks following analysis. Samples that are not retrieved by the client are discarded after two weeks. Preparation of these samples was conducted following protocol outlined in EPA Method SW 846 -3051 unless stated otherwise. Analysis of these samples was performed using analytical instruments in accordance with U.S. EPA, NIOSH, OSHA and other ASTM methods. For matrix materials submitted as paint, dust wipe, soil or TCLP samples, analysis for the presence of total metals is conducted using published U.S. EPA Methods. Paint and soil results are usually expressed in mg/Kg which is equivalent to parts per million (ppm). Lead (Pb) in paint is usually expressed in mg/Kg (ppm) , Percent(%) or mg/cm2 by area. Dust wipe sample results are usually expressed in ug/wipe and ug/ft2. TCLP samples are reported in mg/L (ppm). For air filter samples, analyses are conducted using NIOSH and OSHA Methods. Results are expressed in ug/filter and ug/m3. Other matrix materials are analyzed accordingly using published methods or specified by client. The reported test results pertain only to items tested and are not blank corrected. For recent regulation updates pertaining to current regulatory levels or permissible exposure levels, please call your local regulatory agencies for more details. This report is considered highly confidential and will not be released without your approval. Samples are archived for two weeks following analysis. Samples that are not retrieved by the client are discarded after two weeks.
