On September 20, 2024, Asbesto-Test personnel conducted an Asbestos survey (per
U.S.E.P.A./A.H.E.R.A. . guidelines (as designated and specified by Puget Sound Clean Air Agency
and Washington State) of the burned restaurant located (a, 17216 Smokev Point Blvd., Arlington,
Snohomish County, Washington 98223.This survey's purpose is to identify Asbestos Containing
Materials that are present and will require professional removal prior to a complete tear down.
Note: This structure has burned. The structure is full of contents and debris. This is a limited survey due to the burn,
inaccessibility,unsafe conditions,and contents. There may be additional ACM present that was not discovered in this survey.
If any additional suspect asbestos containing materials are located(prior to or during demolition that were not included in this
survey such as but not limited to those items listed on the last page of this report), those materials are Assumed asbestos
containing unless tested otherwise by laboratory analyses. There should be an AHERA building inspector on site during
demolition to ensure there are no additional suspect asbestos containing materials present that are not identified in this
inspection.
NARRATIVE OF FINDINGS
BASIC CONSTRUCTION: The structure is wood framed, single story restaurant on a concrete slab
foundation. There is also a mezzanine with a burned stairwell (see pictures). The roofing is composition.
The composition roofing, mastics, and roofing vapor barrier materials were sampled. The siding is stucco
with some decorative stone and mortar on various pillars. The stucco siding was sampled. The mortar
holding the decorative stone(which is also present inside the building)was sampled.
INTERIOR CONSTRUCTION, FINISHES, AND FLOORINGS: The interior is finished with wood
and drywall. The drywall/taping materials were sampled. The misc. mastic materials were sampled. The
tile and vinyl floorings, with the relating backing and mastics were sampled. The masonry tiles, with the
mortar and grouts were sampled.

ELECTRICAL SYSTEM: The electrical wiring was not of the type to contain ACM.
HEATING AND VENTILATION SYSTEM: The HVAC (heating/ventilation/air-conditioning unit) is
rooftop. There was no visible suspect ACM located associated with the unit or ducting. Note: It was not
possible to dismantle the HVAC unit and ducting to locate and/or evaluate any additional suspect ACM
which may be concealed, inside the unit, or associated in hidden areas. It is our opinion any further
handling of the furnace system should be coordinated by theproject manager and/or abatement contractor.
If any additional relating suspect materials are locatedprior to and/or during any demolition or renovation,
any and all additional related suspect materials should be considered to be ACM(Asbestos Containing
Material), unless determined to be otherwise by laboratory analysis. Any TSI that may be present inside
or outside the HVAC and/or ducting system including but not limited to all gaskets,joint compounds,
sealants, and/or insulations,are AACM unless tested otherwise by laboratory analysis.
e There is also a decorative gas fireplace in the building. The masonry tile around the fireplace is the same as
what is used as decorative masonry throughout the structure

CONDITIONS
1.All occupants shall be removed from the property prior to any demolition activity.2.Property shall be completely fenced and
secured with chain link fencing being placed on the property lines,ensuring that the parking areas are secured from occupation.3.
Permanent site security fencing shall be installed and maintained eliminating the need for any onsite security trailer.4.An
AHERA certified inspector shall be onsite at all times during demolition/cleanup activity to monitor for Asbestos Containing
Materials(ACM)and ensure that all ACM's are abated and disposed of according to WAC's,RCW's and State L&I
requirements. 5.All demolition materials/debris shall be disposed of at an approved/permitted disposal site i.e. SnoCo Transfer
Station. 6.The demolition contractor shall be responsible for maintaining dust control during the demolition process. 7.A
permanent security fence shall be installed on the property lines after demolition of the building has occurred,if the permanent
fence consists of chain link,then the chain link material must be black vinyl coated. 8.All demolition of buildings shall be
completed within 7 days,and all material/debris removal and site grading shall be completed within 14 days from start of
demolition activity. 9.No piles of material/debris shall be allowed to remain on property,including the building foundation. 10.
Demolition contractor shall be licensed and bonded and shall possess a City of Arlington endorsement prior to any demolition
activity.
The property owner shall ensure that the construction project complies with all applicable zoning codes and regulations.The
property owner shall also ensure that the construction project does not cause any adverse impact on the surrounding environment
or community.The property owner shall be responsible for obtaining all necessary permits and approvals from the relevant
authorities before commencing construction.The property owner shall ensure that the construction project complies with all
applicable design review requirements.
THIS PERMIT AUTHORIZES ONLY THE WORK NOTED.THIS PERMIT COVERS WORK TO BE DONE ON PRIVATE
PROPERTY ONLY.ANY CONSTRUCTION ON THE PUBLIC DOMAIN(CURBS, SIDEWALKS,DRIVEWAYS,

Project Description:
Demolition of burned restaurant building located at 17216 Smokey Point Drive. Building
permit number is BLD-6172
Meeting start time: 10:00 am
Meeting concluded at: 10:12 am
Sean Hedeen noted that there will be a representative from the asbestos reporting agency onsite
during the demolition and construction will stop if anything is seen.
Marc Hayes read through some of the conditions of the permit and asked Sean if he has read them.
Sean responded that he has.
Marc Hayes said that the motorhomes will need to be removed from the property and a security
fence installed on the property line.
Sean Hedeen asked about the temp fence and if it can be put up after demo so that they can get their
equipment and trucks in and out. Marc Hayes responded that the fence will be required during
demo.
Sean Hedeen asked if they need to remove the vegetation. Marc Hayes responded that it could stay.
Marc Hayes told the owner and contractor that they will be allowed 7 days to set the site, clean and
grade it.Sean Hedeen asked if they had to remove the asphalt. Marc Hayes responded that they
don't. Marc said they do have to remove the foundation of the building.
Marc Hayes said it would be best to get all the occupants off the property today and put up the
temporary fence tomorrow.

Marc Hayes asked if locates have been called in.Sean Hedeen responded that they have,and he
believes the utilities have been turned off but not capped yet.
Marc Hayes said they will need to contact the City of Marysville for utility connections.

Description: 1.All occupants shall be removed from the property prior to any demolition activity. 2. Property
shall be completely fenced and secured with chain link fencing being placed on the property lines,
ensuring that the parking areas are secured from occupation. 3.An AHERA certified inspector
shall be onsite at all time during demolition/cleanup activity to monitor for Asbestos Containing
Materials (ACM) and ensure that all ACM's are abated and disposed of according to WAC's,
RCW's and State L& I requirements. 4.All demolition materials/debris shall be disposed of at an
approved/permitted disposal site i.e. SnoCo Transfer Station. 5. The demolition contractor shall
be responsible for maintaining dust control during the demolition process. 6.A permanent
security fence shall be installed on the property lines after demolition of the building has
occurred, if the permanent fence consists of chain link then the chain link material must be black
vinyl coated. 7. Demolition contractor shall be licensed and bonded and shall possess a City of
Arlington endorsement prior to any demolition activity.
