INSPECTOR CERTIFICATION FACILITIES AHERA INSPECTION REPORT 1.0 EXECUTIVE SUMMARY On June 24, 2014, Scott Basom, a Certified AHERA Building Inspector, with the Snohomish County PUD District 1, Facilities Department, conducted a survey for suspect asbestos-containing building materials (ACBM's) in the Arlington Barn at 17601 59th Ave. NE,Arlington,WA. Sampling was conducted at the request of Doug Wilson,prior to Demolition activities. This document summarizes an asbestos inspection complying with the 40 CFR 763 Asbestos Hazard Emergency Response ACT (AHERA). The document may also serve the purpose of a "Good Faith Inspection" report(WAC 296-62-07721). The information in this document needs to be communicated and made available to all contractors and employees performing work in the structure referred to in this report as the Arlington Barn, confined to 3.0 Scope of Work, of this report. 1.1 ASBESTOS Five bulk samples were taken of suspect asbestos containing building materials to determine the presence of asbestos. The samples were sent to RGA Laboratories for testing using polarized light microscopy(PLM). Results of RGA's analysis found no asbestos in any of the five samples. See the attached RGA Lab Analysis and report for details. 2.0 BACKGROUND The purpose of this inspection was to determine if asbestos containing materials(ACM) are present in the building materials on the Arlington Barn at 17601 59th AVE NE,Arlington, WA. The Barn is a wooden structure, in dilapidated condition, of unknown age, is vacant and centrally located on acreage at the above address. The approximate size of the structure is, 32' x 54' x 30' high. Except for roofing materials and a short length of electrical wiring,no other suspect ACM's were observed. 3.0 SCOPE OF WORK The scope of work is to demolish and remove the structure in preparation for construction of a new local office for Snohomish PUD District 1. 4.0 RESULTS Results: Sample identification(ID)numbers were assigned to each building material sample. The sample ID#s begin with an acronym that represents the building, followed by a room#,then a sample number. Each unique material within the room gets a sample number i.e.; wallboard, ceiling tile, floor tile etc. and decimals are used to identify multiple samples taken 1 "FACILITIES AHERA INSPECTION REPORT of homogenous materials. Example: sample ARB-01.1 was taken from the ARlington Barn, material category 01, sample 1,which is wiring insulation connected to a ceiling mounted light fixture, and it is one of multiple samples taken from a homogenous material within the room. Homogenous sampling areas include samples of the homogenous materials found in different rooms or areas. Samples containing more than 1% asbestos are regulated as an asbestos containing material (ACM). If a sample of a homogenous material contains greater than 1% asbestos then all the material in the homogenous sample area is an ACM. Building material containing 1% or less asbestos is considered non-ACM. All five of the samples tested are NON-ACM.. If during the course of renovation or demolition suspect materials are discovered that are not identified in this survey,the materials should be treated as asbestos containing materials until laboratory analysis proves otherwise. 5.0 LIMITATIONS Asbestos inspections are non-comprehensive and subject to many limitations,including those presented below. Our assessment has considered risks pertaining to asbestos;however,this inspection is limited to only those locations sampled.This inspection was not designed to identify all potential concerns or to eliminate all risks associated with demolition,material removal,or construction.This asbestos inspection was performed in preparation for removal of asbestos containing materials in accordance with State of Washington and PSCAA laws and regulations. Materials that are suspected to exist in wall cavities and other hidden spaces are assumed to be asbestos- containing until these areas can be inspected by an AHERA-certified building inspector. The District survey followed work practice and engineering controls for asbestos sampling as specified for Class III asbestos work(29 CFR 1926.1101). The inspector marked the sampling locations on the remodel plans with a unique number corresponding to the sample number to identify the material from which the sample was collected. Materials uniform in type, texture, color, and date of installation are considered to be homogeneous materials (HMATs). Size and pattern changes for materials, such as ceiling and floor tiles,may indicate that materials should not be considered homogeneous. Materials were grouped into HMATs and assigned numbers. An HMAT#is an un-biased, non- repeating, consecutive number assigned by the inspector to numerically represent a specific HMAT. Unique sample numbers were generally developed as follows: Building-HMAT#-HMAT SERIES# A unique building code was given to each of the samples taken on the property. The Arlington Barn samples were was identified as ARB-OX.Y. Where X is the homogeneous material number, and Y is the sample letter or number.Numbers were used to designate samples. Example,ARB-01.1, ARB is Arlington Barn, 01 is the homogenous series and .l is the sample. Suspected ACM samples were collected and stored in heavy-duty, self-sealing plastic bags and shipped through the Operations Warehouse to the RGA Environmental lab in Seattle, Washington. Samples were analyzed via polarized light microscopy(PLM) in accordance with EPA 600/R-93/116, "Method for Determination of Asbestos in Bulk Building Materials." Examination of these samples was conducted for the presence of identifiable asbestos fibers using polarized light microscopy(PLM)with dispersion staining in accordance with U.S. EPA method 600/R- 93/116, "Method for Determination of Asbestos in Bulk Building Materials." For samples containing more than one separable layer of materials,the report will include findings for each layer (labeled L-1 for layer one, L-2 for layer two, etc.). For multi-layered samples, NESHAP requires that layers must be analyzed and reported separately.The asbestos concentration in a sample is determined by visual estimation. For those samples with asbestos concentrations between 1 and 10 percent based on visual estimation, the EPA recommends a procedure known as point counting (NESHAP,40 CFR- Part 61). Point counting is a statistically more accurate means of quantification for samples with low concentrations of asbestos. If you would like us to further refine the concentration estimates of asbestos in these samples using point counting, please let us know. This report is considered highly confidential and will not be released without your approval. Samples are archived for thirty(30) days following analysis. Samples that are not retrieved by the client are discarded after thirty(30) days. This report relates only to the items tested. If samples are not collected by RGA Environmental personnel, accuracy of the results is limited by the methodology and expertise of the sample collector. Analyses are cross-checked with other laboratories for quality assurance purposes.This report shall not be reproduced except in full,without written approval of RGA Environmental. INSPECTOR CERTIFICATION FACILITIES AHERA INSPECTION REPORT 1.0 EXECUTIVE SUMMARY On June 24, 2014, Scott Basom, a Certified AHERA Building Inspector, with the Snohomish County PUD District 1, Facilities Department, conducted a survey for suspect asbestos-containing building materials (ACBM's) in the Arlington Barn at 17601 59th Ave. NE,Arlington, WA. Sampling was conducted at the request of Doug Wilson,prior to Demolition activities. This document summarizes an asbestos inspection complying with the 40 CFR 763 Asbestos Hazard Emergency Response ACT (AHERA). The document may also serve the purpose of a "Good Faith Inspection" report(WAC 296-62-07721). The information in this document needs to be communicated and made available to all contractors and employees performing work in the structure referred to in this report as the Arlington Barn, confined to 3.0 Scope of Work, of this report. 1.1 ASBESTOS Five bulk samples were taken of suspect asbestos containing building materials to determine the presence of asbestos. The samples were sent to RGA Laboratories for testing using polarized light microscopy(PLM). Results of RGA's analysis found no asbestos in any of the five samples. See the attached RGA Lab Analysis and report for details. 2.0 BACKGROUND The purpose of this inspection was to determine if asbestos containing materials (ACM) are present in the building materials on the Arlington Barn at 17601 59th AVE NE,Arlington, WA. The Barn is a wooden structure, in dilapidated condition, of unknown age, is vacant and centrally located on acreage at the above address. The approximate size of the structure is, 32' x 54' x 30' high. Except for roofing materials and a short length of electrical wiring, no other suspect ACM's were observed. 3.0 SCOPE OF WORK The scope of work is to demolish and remove the structure in preparation for construction of a new local office for Snohomish PUD District 1. 4.0 RESULTS Results: Sample identification (ID)numbers were assigned to each building material sample. The sample ID#s begin with an acronym that represents the building, followed by a room#, then a sample number. Each unique material within the room gets a sample number i.e.; wallboard, ceiling tile, floor tile etc. and decimals are used to identify multiple samples taken 1 i I FACILITIES AHERA INSPECTION REPORT of homogenous materials. Example: sample ARB-01.1 was taken from the ARlington Barn, material category 01, sample 1,which is wiring insulation connected to a ceiling mounted light fixture, and it is one of multiple samples taken from a homogenous material within the room. Homogenous sampling areas include samples of the homogenous materials found in different rooms or areas. Samples containing more than 1% asbestos are regulated as an asbestos containing material (ACM). If a sample of a homogenous material contains greater than 1% asbestos then all the material in the homogenous sample area is an ACM. Building material containing 1% or less asbestos is considered non-ACM. All five of the samples tested are NON-ACM.. If during the course of renovation or demolition suspect materials are discovered that are not identified in this survey,the materials should be treated as asbestos containing materials until laboratory analysis proves otherwise. 5.0 LIMITATIONS Asbestos inspections are non-comprehensive and subject to many limitations,including those presented below. Our assessment has considered risks pertaining to asbestos;however, this inspection is limited to only those locations sampled.This inspection was not designed to identify all potential concerns or to eliminate all risks associated with demolition,material removal,or construction.This asbestos inspection was performed in preparation for removal of asbestos containing materials in accordance with State of Washington and PSCAA laws and regulations. Materials that are suspected to exist in wall cavities and other hidden spaces are assumed to be asbestos- containing until these areas can be inspected by an AHERA-certified building inspector. The District survey followed work practice and engineering controls for asbestos sampling as specified for Class III asbestos work(29 CFR 1926.1101). The inspector marked the sampling locations on the remodel plans with a unique number corresponding to the sample number to identify the material from which the sample was collected. Materials uniform in type,texture, color, and date of installation are considered to be homogeneous materials (HMATs). Size and pattern changes for materials, such as ceiling and floor tiles, may indicate that materials should not be considered homogeneous. Materials were grouped into HMATs and assigned numbers. An HMAT#is an un-biased, non- repeating, consecutive number assigned by the inspector to numerically represent a specific HMAT. Unique sample numbers were generally developed as follows: Building-HMAT# -HMAT SERIES# A unique building code was given to each of the samples taken on the property. The Arlington Barn samples were was identified as ARB-OX.Y. Where X is the homogeneous material number, and Y is the sample letter or number.Numbers were used to designate samples. Example,ARB-01.1, ARB is Arlington Barn, 01 is the homogenous series and .1 is the sample. Suspected ACM samples were collected and stored in heavy-duty, self-sealing plastic bags and shipped through the Operations Warehouse to the RGA Environmental lab in Seattle, Washington.
