Addition to existing church w/associated parking area
CONDITIONSt
• Call for required side sewer inspections,Fred Rapelyea 360-913-7058.
• 24 hours prior to start of work contact Marc Hayes at 425-754-7446.

• Call for locates of underground utilities 2 business days prior to any excavation. 1-800-424-5555
• Conform to approved working drawings and Traffic Control plan.
• Maintain erosion&sedimentation control.Keep street clean.
• Construction hours are Monday-Saturday 7am-7pm.No work on Sundays or Federal Holidays.
• Three feet of clearance around fire hydrants,standpipes,P.I.V.'s,manholes,water meters,blowoffs,cleanouts and valves shall be
maintained.
• A Building Permit must be obtained prior to start of work.
• Must follow all City of Arlington Standards and Specifications.

Work to be performed as described and/or shown in the approved plans and/or application for the above named project,is approved
for construction by the City of Arlington Public Works Department. All work is subject to field inspection.

The above named contractor/property owner is herby authorized to perform the above work within City Right-Of-Way in accordance
with the terms of this permit and as shown on the attached drawing and subject to all applicable State, county and/or municipal laws
and/or codes.
All work shall be in accordance with the "Standard Specifications for Road, Bridge and Municipal Construction", except where
modified by City of Arlington Public Works Standards. The contractor shall hold the City of Arlington harmless from any loss or
damage to any person or property in the performance of said work. The contractor agrees to have all restoration of by:
